Overview:

The Director of Care Coordination Services is responsible for overseeing the daily operations of interdisciplinary care teams and ensuring high-quality outcomes for individuals receiving services across various care settings. Reporting to executive leadership, this role manages integrated care delivery across community-based programs, outpatient centers, and home-based services. The Director ensures that care is delivered efficiently, meets regulatory standards, and aligns with organizational goals.

Key Responsibilities

Lead and support site-based managers and clinical staff across multiple disciplines, including therapy, nutrition, social services, and home care.
Ensure balanced team assignments and adequate staffing coverage across all service areas.
Oversee the development and implementation of individualized care plans that promote positive health outcomes and participant satisfaction.
Monitor service delivery and ensure alignment with quality standards and care protocols.
Facilitate staff training and professional development initiatives.
Collect and analyze operational data to support care optimization and cost-effective service delivery.
Participate in quality assurance and compliance activities to maintain required certifications.
Provide administrative support during non-business hours as needed.
Collaborate with internal departments to enhance care models and implement best practices.
Manage participant transitions in and out of the care program and monitor retention efforts.
Address potential disenrollment by implementing proactive interventions.
Ensure accurate and timely documentation across all participant records and care systems.
Investigate and resolve service-related incidents and participant concerns in collaboration with other departments.
Conduct regular chart reviews and implement corrective actions as needed.
Contribute to special projects and organizational initiatives as assigned.
Ensure team compliance with organizational policies and applicable regulations.
Evaluate staff performance and provide ongoing feedback and development plans.
Hold team members accountable for performance and engage HR as needed for disciplinary actions.
Support employee growth through individualized development planning and training.
Address employee concerns and collaborate with HR on resolution strategies.
Participate in departmental budgeting and manage expenditures in line with financial goals.

Qualifications:

Degree from an accredited nursing program required; BSN preferred.
Active and unrestricted Registered Nurse license in the state of New York.
Minimum of one year of clinical experience in home health, acute care, or long-term care settings.
Experience working with older adults or individuals with complex needs preferred.
At least two years of supervisory experience in a clinical setting is strongly preferred.
